Please bring the completed consent form on the day of the event.
Please do not attend if you started having the following symptoms in the Last 24 hours: cough, shortness of breath or difficulty breathing, chills, fatigue, fever, muscle or Body aches, congestion or runny nose, sore throat, headache, new loss of taste or smell, nausea, Vomiting or diarrhea.
If your child has had any of the following he/she will not be eligible to receive a flu vaccine through AESD: egg allergy, history of Guillain-Barre syndrome, or history of a serious reaction to any type of vaccine
