Children must be
3 or 4
years of age by September 1, 2017

Priority is given to children 4 years of age

Families will be pre-screened to determine eligibility. Please fill out the Pre-Eligibility Questionnaire and submit it to the preschool office with the following documents:

  • Immunization Records: Up-to-date for child’s age.
  • Proof of Income: 1 month of recent and consecutive income. All types of income that apply must be submitted. For example: paycheck stubs, cash aid, food stamps, child support, unemployment/disability, SSI, etc.
  • Family Size Documentation: Documentation for all children in the family. Family size documentation may include: birth certificates, hospital registration, passport, CPS letter, Adoption documents, Foster Child/Legal Guardian letter.
